Position Description:
The University of Louisville School of Dentistry is seeking to hire a Medical Case Manager I. The Medical Case Manager will work with Ryan White HIV clients and clinics throughout the state to ensure patients are given competent, timely, and responsible dental care.
Essential Duties and Responsibilities
- Enroll new patients/clients into care at for the Ryan White Oral Health Program
- Assist all Kentucky Care Coordinator Clinics in the state for referrals patients in need of dental care
- Call patients to ensure they make it to their appointments
- Schedule transportation for patients when transportation to appointments is a barrier to care
- Deliver gas cards to patients that drove from outside Louisville to their appointments
- Maintain current knowledge of patients' treatment plans
- Verify that patients are currently enrolled in the RW State Part B grant
- Participate in the collection of data for inclusion in the grant writing process
- Produce weekly updates of scheduled patients having obtained most recent medical labs
- Provide initial assessment of client medication regimen and treatment plan and medical and non-medical service needs; develop a comprehensive individualized care plan; document the coordination of services to implement care plan and monitor the plan to assess efficacy
- Participate in ongoing HIV education and attend Ryan White Part B services conference and seminars
- Travel to state agencies when needed
- Document all client interaction and services provided in the electronic medical record and CareWare database with a high accuracy rate.
- Participate in clinical quality management activities as assigned: CQI point person or CQM point person
- Other duties as assigned
Target Compensation Maximum:
$32.17
Target Compensation Minimum:
$21.45
Compensation will be commensurate to candidate experience.
